Liability Waiver for Grooming

Your Pet is important to us. Because we care, we want to assure you that every effort will be made to make your pet’s visit as pleasant as possible.

Occasionally, grooming can expose a hidden medical problem or aggravate a current one. This can occur during or after grooming. I understand if my pet is severely tangled or matted, it is a greater risk of injury, stress and trauma. All precautions will be taken. However, problems occasionally arise, during or after grooming, such as nicks, clipper irritation and mental or physical stress. I also realize that Senior Pets have a greater chance of injury during grooming. I agree to pay the agreed upon price at the time of drop off, the groomer will notify any changes found during the grooming process.

I agree to notify the groomer of any illness, medical condition and/or behaviors prior to my pet’s groom. This also included specifications for haircuts. I certify that my dog is in good medical health, is up to date on all required vaccinations (Rabies, DHPP, Bordetella). Is illness free and has no signs of vomiting, diarrhea, open wounds, runny eyes, nasal discharge, fever or cough for the last 14 days.

MEDICAL RELEASE AND CONSENT TO TREAT
By signing this form, I hereby grant permission to Briarcliff Pet Resort to obtain emergency veterinary treatment for my pet and authorize the veterinary to resuscitate my pet. I am responsible for all charges incurred including but not limited to deposits to start treatment.

I hereby assume all risks for grooming of my Pet and agree to indemnify and hold harmless the following entities or personas for all claims of liability of any nature whatsoever; Briarcliff Pet Resort any any of its affiliates, subsidiaries, lessees, principals, partners, owners, landlords, officers, employees, volunteers, vendors, agents, representatives. I will not hold Briarcliff Pet Resort responsible for accidents or injury to my pet.

Cancellation Policy
Please give 48 hours notice to cancel or reschedule to avoid being charged the full cost of your grooming appointment.

A no call/no show will be charged the full amount of the appointment with cc info on file. If you cancel the day of your appointment you will be charged the full amount of service.
